First Visit

Upon making a new patient appointment you will receive a link with a request to complete paperwork.



Your initial exam will last approximately 1.5 hours. Oral hygiene instructions will be given along with suggestions designed specially for you to help take care of your teeth. In most cases, you will receive a cleaning at this visit. We will also discuss any existing problems you may have and a treatment plan to correct them and give you the smile you deserve.

Loyalty Program

With our Loyalty Program patients can earn points through the Modento app (this is the app that is used for our electronic forms).


Patients can earn points from referring others, completing paperwork and any required forms prior to your appointment, confiming your appointment, and arriving to your appointment on time.


Some of the rewards you can earn include gift cards to Amazon, Starbucks, Target and products from our office!

Insurance FAQ


Do you accept my insurance?

We are providers for several insurance companies and work with a variety of others. A list of insurances that we are providers for will be coming soon to the website.


I paid my portion of the services I received, but why have I received a bill?

Because of the variations in policies of the dental insurances, we can only estimate your coverage. Some insurance plans have complex contracts making it impossible to give you a guaranteed quote. Also, if you have received treatment in another office before joining our office, we may not be aware of benefits that have already been used.


Why did my insurance not pay?

We will bill your insurance as a service to you. Because of the contract you have with your insurance company, we may not be fully aware of coverages they proved. This coverage varies from company to company and sometimes, within the company. This has nothing to do with the quality of service we provide and the fee charged for these services.
